The term protocols is confusing sometimes because it is used differently from location to location, state to state. In general, protocols have to be very carefully written, or midwives damage themselves legally.

i bought this book and it has been very healing 4 me, i plan to share it with my daughter (paramedic/ambulance driver/1st responder) so she can understand why someone has a different reaction to a normal bio event (like labor or vag examination/assessment during labor) than she expects~its a great book and commend all the women brave enough to share, esp RAINN (rape abuse incest national network)

i 2nd that emotion~i deliberately showed my girls movies of home births (lady & the tramp, clan of the cave bear, moll flanders, xena warrior princess (lucy lawless had home waterbirths in new zealand in rl, both her sons hwb's) and when asked how their father felt about this she said (quote)
"it is not his blood being shed for them" pure Xena::)

Most midwives would love to see a woman before she gets pregnant. It shows the kind of responsibility midwives are hoping for in their clients. And as Jen says nutrition can be talked about. Prenatal nutrition and getting rid of toxins, if possible, are helpful for the new in utero baby. That would be just cleaning house for a new baby since your body is his/her first home!
