Shocking STI stats- Two recent reports have concluded that young women have disproportionately high STI rates:

A 2008 CDC study estimated that nearly 19 million STIs occur each year, almost 1/2 of which are among 15- to 24-year-olds. The heaviest chlamydia and gonorrhea burdens were among adolescent girls, aged 15–19, f...ollowed closely by women aged 20–24. (see link below)

Another study found one in four females aged 14–19 has at least one of the STIs studied (HPV, gonorrhea, chlamydia, trichomoniasis, and herpes). Among female adolescents who said they had had vaginal, oral, or anal sex, the prevalence was 37.7 percent. (http://pediatrics.aappublications.org/cgi/content/abstract/peds.2009-0674v1)

I just finished my Ob/gyn rotation. I saw way too many girls with high grade lesions in their late teens to be convinced to push back all pap tests until 21. Maybe high risk populations could be targeted, as previously with the requirement for pap tests after one became sexually active.

I understand why they have changed their guidelines, to prevent unnecessary treatment in young women in whom most commonly the changes resolve on their own anyways--I am now seeing patients that cannot carry pregnancies without a cervical cerclage due to previous treatment of lesions. BUT I have also seen 15-16 year olds with high grade lesions and those are the ones that need very close follow-up and possibly treatment.
