and at conventions to show the world that what truly matters -- our readers.

I, in consideration of the personal value I may receive from publication of my image, assign and grant to
Dark Horse Comics, Inc. and its Licensor (“DARK HORSE/LICENSOR”), the right and permission to use the
attached photograph. This right is granted for art, advertising, trade or any similar lawful purpose,
and is perpetual.
Accordingly, I hereby release and discharge the DARK HORSE and persons acting for or on behalf of
DARK HORSE from any liability that may arise from or in connection with the use of my likeness.
I hereby warrant that I am of full age, or that if I am a minor that I and my legal guardian have every
right to contract in my name in the above regard, and have the full right and authority
(including copyright) to grant you the above permission. I further state that (I)(we) have read this
RELEASE prior to its execution, and are fully familiar with the contents and agree to all the terms thereof.
