Here's a link to the 2009 Submission Form -- please note it's different than the 2008 form. Sorry the fonts are so small, but choose either PDF or .DOC file.
Either form can be printed out and returned by mail along with submission (don't forget NG and cut scenes!), though the .doc form might be easier if you want to fill it out on a PC and email it back.
