Still haven't got your passport? Need to get it renewed? Come out on Friday November 20th to the Kozlov Centre to
fill out your application or renewal of your passport. For any further questions, please feel free to contact my office
at (705) 726-5959
For first-time passport, you MUST bring:
1. Driver’s Licence or health... card
2. Your original birth certificate (or Can. Citizenship Card)
3. A guarantor – any person with a valid passport who has known you for 2yrs.
*If you do not bring a guarantor that day, they must do three things before your application can be submitted:
Fill and sign the guarantor section of the application form
Sign back of one of your photos
Sign photocopy of your driver’s licence or health card
For renewing your passport:
Your current passport must be near
expiry or has expired within the last year
You only need to bring your old passport to hand in
Come meet your MP!
