Palma Maria Maiorano Here's a challenge for you...it has been for me!: We are using the Viasys system. Like other systems, when you score an apnea or hypop. it will show how much of a desat. followed it. Right? Well, here's the delima that I have discovered in the AASM standards that are related to desats. It states ...the O2 saturati...on is determined by the O2 that is "pre-event".
Palma Maria Maiorano The software basses a desat. on the epoch prior to the event that you mark. Example, Epoch 123 the O2 dropped to 90%, and the pt. has another hypop. and the patient's O2 drops to 89% in epoch 124. It will register as if it is not a true hypop. because it appears to only have a 1% desat.! What I have been doing is wr...iting down the O2 at the time of sleep onset, lets say in the previous scenario it was 94%. Therefore, using that as a baseline/guide as what is a desat. any event that occurs to drop below 91% would be a hypop.
Palma Maria Maiorano But, the way the standards are written (vaguely) I am wrong! Even using my system of writing the baseline O2 down at the time of sleep onset is flawed because if an extremely severe pt. has apneas at the start, his O2 may already be lower than desirable!
