Neglected to answer part of your question- the rewards of being a paramedic are without question invaluable. Extracating someone, safely, from a crushed, burning car, saving a person from an OD, trying to comfort a small child who has just lost his mother to trauma or illness, preventing a person with chest pain from having extensive myocardial ... See Moredamage, etc. is very rewarding. You are also doing most of this work in uncontrolled conditions (not the clean, well lighted ER where you have many collegues present to help out). Try starting an IV in the ER and then try to start one in the back of a moving ambulance and you will see part of what I am trying to convey.

I agree safety comes first. Have always wondered how we can put someone in a KED for skills in 10 mins but supposed to be off scene in 20. I disagree with a standown of HEMS, when you live in a community that difinitive care facilities are 2-3 hours away, we still have a need for helicopters. I have seen patients that spend 3-5 hours in an ER, then 2-3 hours by ground. That is 5-8 hours.
