NEHH is a company I would recommend not only to my parents but to all my family and friends. This company puts the patients care first. Quality of care is very important and our various services help ...not only our patients but their families too. Plus we have the most compassionate clinical staff that enjoy what they do. Philips a leading brand in healthcare technology partners with Northeast Home Health a leading provider in Home Health Care in the North Texas region to expand its reach to critical patients who have a high risk for hospital readmissions.

“We are committed to become the home health of choice in the DFW metroplex by bringing quality health care at home and reduce trips to the hospital.” says Christian Collada. Northeast is providing patients with high risk for hospital re-...admission the Philips Telehealth remote monitoring at home at no cost to them. In addition to the actual nursing visits of their compassionate staff, there will be telehealth equipment at home to monitor patient vitals to be transmitted daily to Northeast’s Case Management team.

According to Eduardo Orosa, “Northeast is raising the standard of care for home health agencies in the area. Agencies who cannot keep up will have a tough time especially with the ACO’s around the corner where outcomes will be clearly measured and monitored.”

This project is part of Northeast’s “Home for 30 Days” campaign to do everything possible through transitional care to prevent hospital readmissions in 30 days.
